Guidelines for Requesting Financial Assistance

- Complete the application.
- Attach any additional information you may have regarding your request such as pamphlets or pricing guides so the committee can better assess each application.
The Officers' Spouses Club Assistance Committee will accept application requests two times during the 2008/2009 fiscal year. All requests must be received by the appropriate deadline to insure consideration by the committee. No post dated requests will be considered (i.e. funds for an event that has already occurred or for an item already purchased).
